- W2000657075 abstract "Introduction: Coeliac disease (CD) was first reported in Asian immigrants almost 30 yrs ago (1). Asian children presented late and were more likely to have atypical features. Since 1975 delaying gluten introduction in weaning diet in the UK has resulted in rise in age at presentation. We compared presenting symptoms, age at presentation and at diagnosis in Asian and White Caucasian children with CD. Methods: Eighty-six patients had histology proven CD diagnosed since 1994. Retrospective review of 73 patients was performed, charts of 13 patients were not available. Fifty-five were White Caucasian, 15 Asians and 3 other racial groups. Fifteen patients had associated diabetes mellitus (DM), 3 were Asian. Small bowel mucosal disaccharidase levels were measured at diagnosis: maltase in 63, lactase in 62 and sucrase in 63 patients. Student t-test was used to compare small bowel disaccharidase levels, age at presentation and at diagnosis; Chi-Square test was used to compare presenting symptoms in the two groups. Results: The mean (SD) age at presentation was 5.0 (4.2) yr in White Caucasian and 7.1 (4.7) yr in Asian patients (p value ns). Mean (SD) age at diagnosis was 5.7 (4.2) yr in White Caucasians and 8.0 (4.5) yr in Asians (p=0.07). Clinical symptoms are shown in the table. Disaccharidase levels were reduced in all the patients at diagnosis but the mean disaccharidase levels were not significantly different in the two racial groups. The mean lactase, maltase and sucrase levels were higher in patients with DM (p<0.01), loose stools and abdominal distension were less frequent. The mean maltase and sucrase levels were significantly lower in patients with loose stools (p<0.05).Table 1: Clinical features of children with coeliac diseaseConclusion: Age at presentation and clinical features of CD in Asian and White Caucasians patients were similar. Asian children were older at diagnosis. Patients with DM had fewer bowel symptoms and disaccharidase levels were higher, suggesting less severe mucosal disease." @default.
