They did not receive any diphosphonates or radiotherapy. Pre-operative evaluation included a CT scan with enhancement using a dye injected intravenously and 3D reconstruction. Associated procedures included lengthening of the brachialis (4 cases), lengthening of the triceps tendon (4 cases) and an anterior capsulotomy in 3 cases. 24 elbows were reviewed with an average follow-up period of 18 months (6 months to 4 years). In 58% of the cases, the result was very good (gain in mobility > 70%) while in the remaining 42% of cases, the improvement was between 40% and 70%. This study confirms the possibility of obtaining good results by excision of the masses of heterotopic ossification of neurogenic origin around the elbow before 1 year after the accident without the necessity of complementary treatment. Vingt patients ont été opérés d'une paraostéo-arthropathie neurogène du coude après traumatisme crânien, entre 1993 et 1997. Il n'y a pas eu de traitement par diphosphonate ni de radiothérapie. L'imagerie pré-opératoire a fait appel notamment à la tomodensitométrie avec injection vasculaire et reconstruction 3D. La chirurgie a été indiquée sur le défaut de mobilité clinique ou sur l'existence de signes cliniques ou électriques de compression nerveuse. Le délai moyen entre l'accident et la chirurgie a été de 34 mois (5 mois à 9 ans). Un allongement du muscle brachial antérieur (4 cas) ou du triceps (4 cas), 3 capsulotomies antérieures ont été associées. Avec un recul moyen de 18 mois (6 mois à 4 ans), 24 coudes ont été évalués. Dans 58% des cas le résultat était très bon (gain de mobilité supérieur à 70%). Dans 42% des cas, l'amélioration était comprise entre 40 et 70%. Cette étude confirme qu'il est possible d'obtenir de bons résultats en excisant une para-ostéo-arthropathie neurogène au coude avant 1 an, sans traitement complémentaire. La evolución promedio entre el accidente y la cirugía fue de 34 meses (5 meses a 9 años). Se asoció a la intervención un alargamiento del músculo braquial anterior (4 casos) o del tríceps (4 casos) y 3 capsulotomías anteriores. Con un seguimiento promedio de 18 meses (6 meses a 4 años), se evaluaron 24 codos. En 58% de los casos el resultado fue bueno (ganancia de movimiento superior a 70%). En 42% de los casos, la mejoría fue entre 40 y 70%. Este estudio confirma que es posible obtener buenos resultados mediante la excisión de una para-osteo-artropatía neurógena del codo antes de un año sin necesidad de asociarle un tratamiento complementario." @default.
