- W2000672723 abstract "Collapsing one lung to facilitate a number of thoracic procedures commonly compromises oxygenation [1, 2]. Application of continuous airway pressure (CPAP) with 100% oxygen to the deflated lung is recognised as an effective method of improving the arterial oxygen tension without interrupting surgery by re-inflating the lung [2]. There are a number of systems described to provide CPAP to the deflated lung [3–5]. However, they are not readily available in our hospital and appear cumbersome. We would like to describe the use of the ‘Ultimate T–Bag’ (Intavent Orthofix Ltd.) as a lightweight method for improving oxygenation during single lung ventilation. The ‘T–Bag’ is an injection moulded, polyethylene T-piece and shaped reservoir bag of 300 ml volume [6]. It currently has widespread use as a means of providing supplemental oxygen via the laryngeal mask airway during the initial recovery period [6]. During anaesthesia for the surgical management of an aneurysmal bone cyst involving the fifth thoracic vertebra, prolonged deflation of the patient's right lung was required. On changing to single lung ventilation the patient's saturations dropped to 87% despite 60% inspired oxygen. We attached a ‘T-bag’ to the right lumen (deflated lung) of a left-sided double lumen tube and connected this to a separate oxygen cylinder via standard green oxygen tubing (Fig. 6). The flow meter on the cylinder was set to 2 l.min−1 and some tape placed over the vent for the inspired and expired gases of the ‘T-Bag’ to keep the bag distended and provide some positive pressure to the deflated lung. Importantly, the tape was pierced in the centre to allow oxygen to escape, otherwise the right lung would have re-expanded and been exposed to increasing pressure. The patient's lung remained deflated for a 2-h period with no interruption to surgery. Throughout this time, the patient's saturations remained at 98% and the surgeons reported no tension on the deflated lung." @default.
