However, there is little data addressing these symptoms in patients after urethroplasty. This study examines the spectrum of LUTS after urethroplasty for anterior USD and the relationship of these symptoms to USD recurrence. METHODS We performed an IRB approved retrospective review of our urethroplasty database to collect demographic, stricture related, and post-operative information with emphasis on patient reported LUTS. Irritative symptoms (IS) were defined as frequency, urgency, nocturia. Obstructive symptoms (OS) were defined as weak stream, straining, intermittency, and incomplete emptying. Fisher’s exact test was used to compare LUTS with respect to recurrence and USD location. RESULTS We identified 549 men who underwent anterior urethroplasty by two surgeons (GDW, ACP) from January 1996 to September 2011. 475 (87%) men had follow up data available to determine recurrence. Of these 475 men, 433 (91%) had a successful outcome from urethroplasty with no recurrence with a median follow up time of 10.4 months. Of the 433 men, only 41 (9.5%) reported LUTS, most commonly urinary dribbling (21, 4.9%) and spraying of urinary stream (9, 2.1%). The majority of patients without recurrence were asymptomatic versus those with recurrence (95 vs 2.4%, p<0.001). 42 men (8.8%) developed recurrence at a median time of 19 months (range 1-141 months). The most frequent LUTS complaint included weak stream (37, 88.1%) and dribbling (2, 4.8%). The symptom most significantly associated with recurrence was weak stream (p<0.001), however, two symptoms not included in the AUA-SI, hematuria and dysuria, were also significantly associated with recurrence. There were no major differences among failures when stratified by USD location (Table 1). CONCLUSIONS Our findings indicate that men with a successful outcome after urethroplasty tend to remain asymptomatic whereas those who recur have LUTS. The most frequent is weak stream, but two symptoms not included in the AUA-SI, dysuria and hematuria, are significantly associated with recurrence. This supports the need for a USD specific validated questionnaire to be used for follow up after urethroplasty. Table 1.
