- W2000766710 abstract "The aim of this paper was to correlate the preoperative and postoperative findings in Morton's syndrome to establish guidelines for surgical decision-making. Thirty-two Morton's operations were reviewed and preoperative criteria, intraoperative findings and histological specimens compared. The patient population formed a very homogeneous group with respect to the site of lesion in the third interspace, clinical and physical findings, and radiological imaging. Four patients had a simple nerve entrapment that required only surgical decompression via neurolysis. Five patients had an enlarged intermetatarsal bursa with perineural and endoneural fibrosis. Seven patients had a globous nerve enlargement with sclerosis of perineurium and fibrosis of endoneurium. In 17 cases the nerve was found to be macroscopically enlarged, with a true neuroma-like appearance. The microscopical changes were typical of a neuroma. The authors consider Morton's syndrome to be an entrapment syndrome only in the early stages, when the clinical signs are suggestive but sonography or magnetic resonance imaging (MRI) of the intermetatarsal web spaces are negative. In more advanced cases, when the intrafascicular connective tissue in an apparently normal nerve starts to proliferate, neurectomy is advised.
