The mainstay for treating low- to mid-grade strains has traditionally been conservative management, the goal of which is to restore the athlete back to his or her previous level of activity without predisposing them to future injury. This includes the RICE protocol (rest, ice, compression, elevation), activity modification, and gradual rehabilitation. Surgical management is rare and usually reserved for complete avulsion injuries either proximally off the ischial tuberosity or distally from the proximal fibula or tibia.
