• W2000873844 abstract "血管撮影による止血治療法は,手術的止血に比べると患者への侵襲が少なく,かつまた治療技術の進歩に伴ってその成績も向上しているので救急領域においても重要な役割を演じている。動脈性消化管出血に対する血管撮影による止血治療のよい適応は,手術的止血に高い危険を伴う多臓器不全あるいは多臓器不全への移行が危惧される患者,小腸出血の患者,大量出血で内視鏡で診断と治療をすることが困難な患者,重症膵炎患者などである。一方,外傷性出血は,救命のために一刻も早く患者を救急室から手術室に移動し,外科的に処置しなければならない場合を除くと,これらの患者の多くが血管撮影による診断と治療の適応となりうる。骨盤骨折に伴う後腹膜出血は血管撮影による止血治療法による止血成績も良好で,かつ血管撮影でこれら患者に併発することが多い腹部大動脈とその分枝の血管損傷の診断と,症例によっては治療もできるので,血管撮影による止血治療法が第一選択となっている。また最近は,肝臓,腎臓,脾臓などの腹部実質臓器の動脈損傷に対しても本法が積極的に試みられ,良好な成績が得られている。血管撮影による止血治療法によって起こる臓器梗塞などの重篤な合併症の報告は初期に多くみられたが,技術の進歩に伴って著しく減少し,現在は安全な治療法となっている。救命率のさらなる向上のために救命救急システムの見直しと再構築が検討されているなかで,わが国においても最新の画像診断とinterventional radiologyに精通したangiographerが救命救急チームのスタッフとして貢献できるシステムの確立が期待される。" @default.
