In this way, the gas monitoring line can be connected to the breathing circuit just once whenever a new circuit is set up, and remain attached for the whole of the circuit's life, which may possibly be up to seven days if departments follow some of the manufacturers’ recommendations 3. This configuration completely eliminates the need ever to interrupt the sampling line connection at all, such as occurs when connecting and disconnecting from the breathing filter in the anaesthetic room and then reconnecting in theatre, if the anaesthetist chooses to use a sampling port on each patient's single-use breathing filter. Following the Expert Group on Blocked Anaesthetic Tubing (EGBAT) report 4, most monitoring ports now have tethered caps, but having any sampling ports on breathing filters still presents unnecessary hazards. Almost every anaesthetist will have experienced leaks from breathing filter sampling ports at some point in his/her career. The time and historical reason for the location of these ports has now passed 5, and if anaesthetists follow the new AAGBI guidelines this hazard can be removed with a small change in practice back to what everyone did before they were introduced. Human factors dictate that the natural inclination is to put the monitoring line wherever there is a port, even if it is not the best or safest place to put it. Our colleagues in industry have an important role to play here, and to free anaesthetists of this temptation they should now remove all sampling ports from breathing filters and return to their original design without any gas sampling ports. To support this system improvement, anaesthetists should invoke the National Patient Safety Agency's ‘purchasing for safety’ policy and always use plain breathing filters without ports, and assemble the gas monitoring line as an integral part of the breathing circuit on the non-patient side of the filter, thus following the AAGBI guidelines. Currently, this can easily be done using a T-piece connector (Fig. 5). Manufacturers of breathing circuits, for their part, should start to include ports at the distal ends of their circuits or fully incorporate the gas monitoring line into the breathing circuit, as some already do." @default.
