• W2000898660 abstract "To assess the correlation between the timing of cervical shortening and the risk of spontaneous preterm delivery (SPTD) in twin gestations. Retrospective review of twin gestations followed from 1994-2003 at a single institution. Patients who underwent transvaginal ultrasound (TVU) cervical length (CL) measurements between 16 and 23 6/7 weeks (w) gestational age (GA) were eligible. Exclusion criteria were prophylactic cerclage, lack of TVU measurements, loss to follow up, or medically indicated preterm delivery (PTD). Those patients who had a CL <25 mm were stratified according to GA at the time of cervical shortening. The odds ratio (OR) for SPTD <32 w in those with CL <25 mm at 16-19 6/7 w GA was compared to those with CL <25 mm at 20-23 6/7 w. Mean GA at delivery for those with CL of <25 mm at 16-196/7 w and at 20-23 6/7 w were compared. Of the 91 twin gestations identified, 2 were excluded for prophylactic cerclage, 2 for lack of delivery data, 3 had no TVU, 2 had not yet delivered, and 1 had a medically indicated PTD. Of the 81 remaining twins, 12 had CL <25 mm. Two of 3 patients with CL <25 mm at 16-19 6/7 w had SPTD <32 w compared to 5 of 13 patients with CL <25 mm at 20-23 6/7 w (see Table). Mean GA at delivery for CL <25 mm at 16-19 6/7 w and 20-23 6/7 w were 26.9 w and 31.3 w, respectively, P = .263. The OR for SPTD <32 w was approximately three times higher and mean GA at delivery was 4 w earlier in twins with CL <25 mm at 16-19 6/7 w compared to 20-23 6/7 w.
