- W2000975679 abstract "La sarcopénie, caractérisée par la fonte des réserves protéiques musculaires, est fréquente chez le patient âgé, mais touche aussi l’obèse. Ses mécanismes sont complexes. Il est établi que la réponse anabolique postprandiale est diminuée du fait d’une augmentation de l’extraction splanchnique des acides aminés et d’une résistance du tissu musculaire à l’effet des acides aminés et de l’insuline. Différentes stratégies nutritionnelles ont été évaluées pour développer des effets anabolisants au niveau musculaire par une stimulation de la protéosynthèse et/ou une diminution de la protéolyse. Elles reposent sur la concentration de plus de 60 % des apports protéiques journaliers sur un seul repas, sur l’apport d’acides aminés spécifiques (leucine, citrulline) ou encore l’utilisation de protéines dites rapides permettant d’améliorer l’hyperaminoacidémie postprandiale. Une autre approche possible est l’utilisation de pharmaconutriments (acides gras polyinsaturés de la série n-3, glutamine…) pour moduler l’inflammation systémique et l’insulinorésistance observées au cours du vieillissement et limiter ainsi le déficit protéique. Sarcopenia is characterized by the depletion of muscle protein pool and is frequently observed in elderly but also in obese patients. The underlying mechanisms of sarcopenia are complex. It is well documented that the postprandial anabolic response of the muscle is blunted consequently to an increased splanchnic extraction of amino acids and to the resistance of muscle to the anabolic effects of amino acids and insulin. Optimized nutritional strategies have been developed to improve muscle protein metabolism by increasing protein synthesis and/or decreasing proteasome-mediated proteolysis: concentrating a high amount of nitrogen intake (> 60% of daily intake) in one meal, supplying specific amino acids (leucine, citrulline) or providing “fast” proteins to increase postprandial hyperaminoacidemia. An other possible strategy could be to target systemic inflammation or insulin resistance observed during aging through the use of other pharmaconutrients, that is polyunsaturated fatty acids n-3 or glutamine." @default.
