They assert that a warfarin dosing nomogram achieves the same degree of protection from VTE disease by day 30 in hip and knee arthroplasty patients when compared with house staff dosing orders. Their study only examines the first 4 to 6 days of warfarin therapy. Although the average international normalized ratios of the 2 groups are remarkably similar in those first few days, they did not examine the adequacy of the full course of warfarin therapy because this occurred after discharge and was monitored by different clinicians. There is a great difference between apparently equivalent initiation of warfarin therapy and true outcome effectiveness." @default.
