Plusieurs auteurs ont expliqué par une allergie à ce produit certains accidents post-opératoires cutanés ou profonds. Nous rapportons l'observation d'une jeune fille qui depuis une appendicectomie présente des éruptions érythémato-bulleuses récidivantes localisées à l'abdomen, toujours précédées de douleurs coliques droites et de troubles du transit. Un lien de causalité ayant paru évident entre l'intervention chirurgicale et cette dermatose, les recherches étiologiques nous ont amenés à incriminer le catgut. Notre hypothèse paraît confirmée par la forte positivité des tests cutanés accompagnée de réactions focales reproduisant les lésions et la positivité du test de transformation lymphoblastique. Réputé disparaître rapidement de l'organisme, le catgut semble donc pouvoir se comporter comme un allergène persistant et provoquer des réactions au long cours. Des tests simples permettent de vérifier la sensibilisation à ce produit. Ils devraient être effectués avant toute intervention chirurgicale chez les sujets allergiques et les malades ayant déjà été opérés. Un autre matériel de suture devrait être utilisé en cas de réaction positive en attendant la découverte d'un fil résorbable véritablement non allergisant. Catgut, the material used for surgical stitching, is a substance of animal origin that can behave as an allergen. Several authors have explained cutaneous or deep postoperative accidents in terms of an allergy to this material. The authors report the case of a young girl who since an appendectomy has presented localized recurring erythematous blistering of the abdomen, always preceded by right colic pains and transit difficulties. As there seemed to be a causal link between the operation and the dermatosis, etiological enquiries led the authors to incriminate catgut. The hypothesis appeared to be confirmed by the strong positivity of cutaneous tests accompanied by focal reactions reproducing the lesions and by positivity of the lymphoblastic transformation test. Although catgut is reputed to disappear quickly from the body it seems to behave like a persistent allergen and to provoke long-term reactions. With simple tests it is possible to check sensitization to this material. This should be done before all surgical operations in allergic subjects and in patients who have been operated on previously. Another suture material must be used in patients with positive reactions until a resorbable thread is discovered that is truly non allergenic." @default.
