- W2002686010 abstract "RATIONALE: Assess the frequency of patient reported adverse reactions (PRAR) to antibiotics in preoperative questionnaires and selection of antibiotic for antibacterial prophylaxis perioperatively (ABPPO). METHOD: IRB approved study for retrospective review of the medical records of patients operated at our institution from February 23-27 and April 6-12, 2004. RESULTS: In 426 individual patient records of surgical procedures, the frequency of PRAR to one or more antibiotics was 130/426 (30.5%) with 50/426 (11.7%) to penicillin, 10/426 (2.3%) to amoxicillin/ampicillin, 19/426 (4.5%) to cephalosporin and 51/426 (12%) to sulfa. The surgical team chose the antibiotic for ABPPO. No allergy evaluation or testing was done. Overall 305/426 (71.5%) of the patients received cefazolin for ABPPO. In the group of patients with PRAR to penicillin, amoxicillin and ampicillin, 23/60 (38%) received cefazolin, 15/60 (25%) vancomycin and 22/60 (36%), to clindamycin, ciprofloxacin, metronidazole and gentamycin. Of those with PRAR to cephalosporin 1/15 (6.6%) received cephalosporin 5/15 (33.3%) vancomycin and 9/15 (60%) received other antibiotics. No adverse reactions were noted. Four patients without PRAR to antibiotics had reactions perioperatively. Two patients who developed hives were given cefazolin and 2 patients who developed hypotension one received cefazolin and one vancomycin. CONCLUSION: In this study setting PRAR to antibiotics in preoperative questionnaires was high. For ABPPO the majority of patients with or without PRAR to penicillin or any of the beta lactam antibiotics received cephalosporin without adverse affects. A significant number of patients with PRAR to penicillin received vancomycin." @default.
