- W2002753676 abstract "Amniotic fluid (AF) sludge is a risk factor for preterm delivery and intra-amniotic infection in patients with preterm labor and intact membranes. However, the prevalence and clinical significance of AF sludge in asymptomatic patients at risk for preterm delivery is unknown. 281 asymptomatic singleton pregnancies at risk for preterm delivery (history of spontaneous preterm delivery, cervical insufficiency, conization, or mid-trimester short cervix) underwent cervical sonography between 13 to 29 weeks. Outcomes included microbial invasion of the amniotic cavity (MIAC), histologic chorioamnionitis, ultrasound-to-delivery interval, preterm PROM (PPROM), and spontaneous preterm delivery < 28, < 32, and < 35 weeks. Data were analyzed using contingency tables and multivariate logistic regression. 1) The prevalence of AF sludge was 23.5% (66/281); 2) patients with AF sludge had a higher frequency of MIAC [(20% (9/45) vs. 1.5% (1/65), p = 0.001] and histologic chorioamnionitis [61.5% (40/65) vs. 29.5% (57/193), p < 0.001] than those without sludge; 3) patients with AF sludge had a higher rate of spontaneous preterm delivery than patients without sludge [< 28 weeks: 43.5% (20/46) vs. 5.3% (9/169), p = 0.001; < 32 weeks: 54.3% (25/46) vs. 12.4% (21/169), p = 0.001; and < 35 weeks: 63% (29/46) vs. 19.5% (33/169), p = 0.001]; 4) patients with AF sludge had a shorter ultrasound-to-delivery interval than those without sludge [AF sludge median: 8.1 weeks (0.1–24.4 weeks) vs. no sludge, median: 14 weeks (0.2–25.2 weeks); p < 0.001]; and 5) Multivariate logistic regression analysis indicated that AF sludge was an independent risk factor for spontaneous preterm delivery, PPROM, MIAC, and histologic chorioamnionitis (Table). The presence of AF sludge in asymptomatic high-risk patients is an independent risk factor for spontaneous preterm delivery, PPROM, MIAC, and histologic chorioamnionitis, and confers an odds ratio of 8 for delivery at < 28 weeks." @default.
