- W2003117525 abstract "78歳，82歳という高齢発症SLE（LO-SLE）が疑われる2症例に合併した血球貪食症候群（HPS）を経験した．2例とも食思不振を主訴とし，SLEに特徴的な症状は無かった．明らかな感染の認められない発熱，血球減少，胸膜炎，心膜炎，肝障害，血液凝固異常などを呈し，骨髄穿刺にて血球貪食組織球を認め，自己抗体の存在よりHPS合併LO-SLEに矛盾しないと判断した．一例は中等量のステロイドに反応し全身状態および検査所見の改善が得られたが，他例は診断時にすでに全身状態が悪化しており，ステロイドパルス療法を施行したにも関わらず，肺炎の併発により死亡した．SLE関連HPSの多くは免疫抑制療法に反応し，その予後は他の原因によるHPSよりも良好であるとされるが，高齢者の場合は診断時すでに全身の衰弱状態にあることが多く，早期の診断と治療が不可欠であるため，LO-SLEの存在とさらにHPSの合併を常に念頭に置いて診断にあたる必要がある．" @default.
