- W2003121446 abstract "目 的 産褥期における腰痛の経日的変化と関連する要因を明らかにすることを目的とした。対象と方法 初産の褥婦を産後1日・5日・1ヵ月の3回追跡し，構造式面接調査およびアンケート調査を実施した。結 果 産後1ヵ月まで追跡できた褥婦67名を分析の対象とした。腰痛保有率は妊娠末期58名（86.6％），産後1日31名（46.3％），産後5日35名（52.2％），産後1ヵ月32名（47.8％）であった。腰痛は産褥早期に5割程度まで有意に減少したが（p＜0.001），それ以降の産褥1ヶ月間は有意な減少はないまま腰痛は残存した。 産後5日の腰痛強度は妊娠中の体重増加量が多いほど強かった（r＝0.392, p＜0.01）。産後5日の腰痛による日常生活困難度は，分娩期の身体的な困難度（r＝0.381, p＜0.01），会陰切開・会陰裂傷による日常生活の困難度（r＝0.513, p＜0.01）が大きいほど大きかった。また産後5日の腰痛保有者は妊娠中に軽労働の就労に従事していたものほど少なかった（p＜0.05）。産後1ヵ月の腰痛強度は体重減少率が小さいほど強かった（r＝0.336, p＜0.01）。結 論 腰痛の多くは産褥早期に減少したが，産後1ヵ月経過してもなお40％以上の褥婦に腰痛が残存した。妊娠中の適正以上の体重増加，分娩による身体的負荷（仰臥位の持続と会陰損傷），産褥期の体重減少率の低さが産褥期の腰痛残存に関連していた。" @default.
