- W2003275267 abstract "Intravenous diuretic treatment of volume overload in patients with heart failure is associated with no change or an increase in body weight at the time of hospital discharge in approximately 30% of patients entered in a large registry. This suggests that diuretic therapy itself or the endpoints for its discontinuation is ultimately ineffective in volume management. To investigate the mechanisms underlying this observation, changes in plasma volume were estimated using deidentified data obtained from 366 patients admitted to The Ohio State University Medical Center for management of heart failure and volume overload and who received intravenous diuretics. Changes in plasma volume were estimated by change in hematocrit (Hct) according to the formula: (100/100-Hctpre))∗(100(Hctpost-Hctpre)/Hctpost) where Hctpre is baseline and Hctpost is Hct at the end of diuretic therapy. As shown in the Table, 43.7% of patients had an increase in plasma volume at the conclusion of diuretic therapy and in 25% of those, the increase was between 20 to 80%. Plasma BNP concentrations were available in a subset of these patients. Those with a decrease in plasma volume were found to have a decrease in BNP (pg/ml) from 1146 ± 140 to 1088 ± 139 but those with an increase in plasma volume were found to have an increase in BNP from 992 ± 170 to 1177 ± 187 (intergroup p = 0.06). The proportion of patients in this analysis having an increase in plasma volume closely parallels the reported proportion of patients with no change or an increase in body weight following diuretic therapy. The increase in estimated plasma volume with diuretic therapy may reflect functional renal impairment or reduction in vascular hydrostatic or oncotic pressures.
