- W2003287653 abstract "We wish to congratulate Drs. Mehanna and Jankovic on an interesting and erudite review [ [1] Mehanna R. Jankovic J. Respiratory problems in neurological movement disorders. Park Rel Disord. 2010; 16: 628-638 Abstract Full Text Full Text PDF Scopus (97) Google Scholar ] on respiratory dysfunction in movement disorders. We wish to add our own experience with shortness of breath in people with Parkinson’s disease, since it supplements the observations of the articles they cited. Shortness of breath in Parkinson’s disease has been ascribed to: obstructive upper airway disease [ [2] Sabate M. Rodriguez M. Mendez E. Enriquez E. Gonzalez I. Obstructive and restrictive pulmonary dysfunction increases disability in Parkinson’s disease. Arch Phys Med Rehab. 1996; 77: 29-34 Abstract Full Text PDF PubMed Scopus (71) Google Scholar ] restrictive problems [ [2] Sabate M. Rodriguez M. Mendez E. Enriquez E. Gonzalez I. Obstructive and restrictive pulmonary dysfunction increases disability in Parkinson’s disease. Arch Phys Med Rehab. 1996; 77: 29-34 Abstract Full Text PDF PubMed Scopus (71) Google Scholar ], respiratory dyskinesias, autonomic dysfunction during “off” periods [ [3] Khan W. Naz S. Rana Q. Shortness of breath, a “wearing-off” symptom of Parkinson’s disease. Clin Drug Investig. 2009; 29: 689-691 Crossref PubMed Scopus (7) Google Scholar ], anxiety and possibly carotid body dysfunction due to dopamine containing glomus cells [ [1] Mehanna R. Jankovic J. Respiratory problems in neurological movement disorders. Park Rel Disord. 2010; 16: 628-638 Abstract Full Text Full Text PDF Scopus (97) Google Scholar ]. We have accrued 15 patients with idiopathic PD who were referred for pulmonary function studies after cardiac evaluation found no evidence of a cardiac related breathing problem. In no case were dyskinesias a significant problem. They did not have the spells during “off” periods and were not deemed anxious. Four of the 15 patients had evidence of mild obstruction, none had restrictive disease and none had pulmonary function tests that explained their dyspnea. One had mild shortness of breath at baseline which worsened dramatically with each dose of l-Dopa. Thus, none of them had explanations found in the literature." @default.
