- W2003670088 abstract "Epidemiological studies have produced important information about multiple sclerosis (M.s.) although the aetiology of this disease remains obscure. There has been no apparent change in M.S. rates over time in the few areas where solid data are available. The rates of M.S. in the northern hemisphere increase from the equator to the north temperate zones. The rates at a given latitude vary, however, implying that latitude is not the sole determinant. Information from migrant studies suggests that adult migrants from high-risk areas to low-risk areas have a higher rate of M.S. than those born in low-risk areas, but for the most part their rate is not as high as in the high-risk area. There are some data indicating that adults migrating from low-risk areas do not develop rates as high as those in high-risk areas, which implies some form of protection and raises the possibility of preventing M.S. Evidence is accumulating that the most important risk determinants occur around adolescence. The age-of-onset curve is almost unique, with a sharp peak about age 30 and very few new cases before age 15 or over age 55. Sex differences are not striking. Other than latitude, no consistent environmental factors have been documented, but with the recent evidence that critical factors occur around adolescence it is possible that studies of events in this period would be revealing. Measles-antibody titre is slightly higher among M.S. patients than controls, but the epidemiological implications of this are obscure, since the geographical patterns of M.S. and measles are dissimilar. An infectious ætiology of M.S. is an attractive hypothesis in view of the geographical distribution, the data on migrants, and the age-of-onset curve. The observed patterns, however, are not compatible with models of simple virus infections or slow viruses and are best explained in terms of two distinct determinants, one causal and one preventive." @default.
