Only 3 cases of an infected hydrocele or pyocele have been previously described in the neonatal period.l-3 We report on a 3-day-old newborn who presented with acute scrotal swelling that proved to be an infected hydrocele on surgical exploration. CASE REPORT A 3-day-old neonate presented to the emergency room with acute onset of right scrotal swelling. The patient was born at term by normal spontaneous vaginal delivery. He was discharged home on day 1 after birth following a normal physical examination by an attending pediatrician and pediatric resident. On day 3 of life the mother noticed swelling of the right testicle and irritability when it was touched. On physical examination the newborn was afebrile. The right testicle was swollen and indurated with skin erythema and tenderness to palpation. The remainder of the examination was unremarkable. Laboratory evaluations included normal complete blood count and urinalysis. Color Doppler ultrasound revealed a right paratesticular mass and high flow within the right testicle (part A of figure). Hydrocele fluid with debris was present around the right testicle, suggesting the possibility of an infectious process. Exploration was performed via a scrotal incision. A markedly thickened tunica vaginalis was identified containing approximately 3 cc of purulent material. The testicle and epididymis appeared healthy with mild erythema.
