- W2004052877 abstract "Several observational studies found that a prior higher degree of physical activity is related to a subsequent lower dementia incidence, although this association is still not definitive. Aim of this study is to investigate the association between walking habits in late middle age and presence of dementia at 100 years. The Monzino 80-plus Study is a prospective population based study being conducted in eight municipalities of Varese province (Italy) on the epidemiologic characteristics of dementia. All participants, together with their proxy respondents, received a thorough interview and cognitive testing. We extended the survey to centenarians in the whole province of Varese to have a sufficient number of subjects in this age group. Walking habits 40 years ago (how often they went out for a walk) were also investigated. Subjects reporting walking at least once a week were defined as “frequent walkers”. Diagnosis of dementia was based on DSM-IV criteria. The great majority (92%) of the 245 centenarians were women. Centenarians “frequent walkers at 60 years” had clinical conditions similar to “non-frequent walkers at 60 years” but had slightly better mood conditions and abilities in activities of daily living. Prevalence of dementia at 100 years was much lower among “frequent walkers at 60 years”: 54% versus 82% (p < 0.0001). This difference remained significant when corrected for sex, age and education (OR: 3.7, 95%CI: 2.0-7.0), and also when possibly intervening clinical conditions were added to the model: stroke, chronic obstructive pulmonary disease, diabetes, hypertension, history of myocardial infarction and history of depression (OR: 3.8, 95%CI: 2.0-7.3). These relationships were confirmed (p < 0.0001) when we used, instead of the “frequent” and “non-frequent” definitions, the original narrower six categories related to frequencies of walking (from “never walked” to “walked every day”;). Walking in late middle age is associated with a lower prevalence of dementia in those who reach 100 years of age. This result confirms those of previous studies showing an association over shorter periods of time and in younger populations." @default.
