- W2004231966 abstract "Coronary artery disease (CAD), cerebrovascular disease, and hypertension (HTN) occur in approximately 68 million Americans.1 Of the approximately 27 million patients who undergo noncardiac surgery each year, about 8 million have documented or multiple risk factors for CAD and about 4 million are over 65 years old.48, 49 Ambulatory surgery, currently greater than 50% of all surgical procedures in hospitals in the United States, is predicted to grow in volume. Healthcare reform and cost containment have contributed to a shift of surgery from inpatient to outpatient status. Each year brings more complex patients and more extensive surgery to the ambulatory arena. Thus, it is very likely that patients presenting for elective ambulatory surgery will also have some degree of heart disease. The evaluation and management of this group of patients for ambulatory surgery presents a major challenge. Numerous studies have attempted to identify risk factors for perioperative cardiac complications. Almost all of these studies, however, have been performed on inpatients undergoing high-risk surgery. Until more studies are performed on patients undergoing ambulatory surgery, it will be necessary to extrapolate inpatient data to outpatient situations. Quality assurance review of ambulatory surgery may be one of the only methods of providing information regarding perioperative cardiac complications. This method may help to determine which patients and which procedures can be safely performed as ambulatory surgery. The challenge to the anesthesiologist is to apply this information and develop treatment strategies for the patient with cardiac disease undergoing ambulatory surgery. The specific problems of preoperative evaluation, anesthetic management, and postoperative follow-up in the patient with CAD for ambulatory surgery are addressed in this article." @default.
