- W2004349143 abstract "This letter is in reference to the article “Composite Film and Graft Unit for the Recipient Area Dressing After Split-thickness Skin Grafting in Vitiligo” by Subrata Malakar and Rita Shah Malakar.1Malakar S Malakar RS. Surgical pearl: composite film and graft unit for the recipient area dressing after split-thickness skin grafting in vitiligo.J Am Acad Dermatol. 2001; 44: 856-857Abstract Full Text Full Text PDF PubMed Scopus (8) Google Scholar In their article, the authors have rightly stated that it is very important to avoid movement of the graft over the recipient bed to prevent wrinkling and peripheral beading and have advocated the use of a transparent film and graft unit for this purpose. Furthermore, the authors have mentioned that the conventional dressing for the recipient area consists of antibiotic ointment, sterile gauze, and Micropore tape. In this context, I would like to mention flexible collodion as a dressing for the recipient area. Flexible collodion has been used since 19642Behl PN. Treatment of vitiligo with homologous thin Thiersch's grafts.Curr Med Pract. 1964; 8: 218-219Google Scholar to obtain effective fixation of the graft and has proved extremely effective in preventing the complications of beading and the halo effect mentioned by the authors. In this method, a thin layer of cotton is spread over the recipient site after the graft has been placed. It is important that the graft be 2 to 3 mm larger than the recipient site to prevent shrinkage. Flexible collodion is then applied with a cotton-tipped swab over the cotton layer, which forms an effective seal to provide adequate stability and prevent movement of the graft. The total area of the cotton seal is at least 0.5 cm larger than the area of the graft. A few punctures with a sterile needle are made in the seal to allow drainage of any oozing fluid. The seal is removed 5 to 7 days later, after it has been wetted with acetone or saline solution. The seal peels off easily without any damage to the underlying graft. The edges of the graft are trimmed with scissors. Use of flexible collodion is thus an effective and inexpensive method for dressing the recipient site3Behl PN. Vitiligo surgery.in: Practice of skin surgery. 1st ed. Skin Institute and School of Dermatology, New Delhi1990: 81-86Google Scholar and has been used routinely in India. I have also used other sprays, such as Opsite spray (Smith and Nephew, United Kingdom) with equal efficacy5Mysore V. Punch grafting in vitiligo.Gulf J Dermatol. 2000; 7: 27-31Google Scholar in place of flexible collodion." @default.
