- W2004378520 abstract "Toxemia of pregnancy is seen generally in late pregnancy, with an acute onset of hypertension, edema, proteinuria, and various neurological abnormalities, including seizure and coma. The differential diagnosis of seizure and coma in late pregnancy involves eclampsia, epilepsy, intracerebral hemorrhage, cerebral sinus and venous thrombosis, and thrombotic thrombocytopenic purpura. The present authors show the results of computerized tomography scanning in two cases. Case 1. A 28-year-old woman (G3P3Ab0) experienced a generalized seizure 10 minutes after spontaneous vaginal delivery. Examination following the seizure disclosed hypertension, and over the next few hours, a profound edema and significant proteinuria developed. Despite medical therapy for eclampsia, a second seizure occurred approximately 12 hours postpartum. Two days later, examination showed physical findings consistent with toxemia. Tomography scanning, performed before and after the use of contrast material, revealed increased intracranial pressure, demonstrated by obliteration of the sylvian and basilar cisterns, with a slit-like third ventricle and loss of the normal cortical sluci. More prominently, there were scattered low densities involving the white matter of the occipital, parietal, and left temporal lobes, and low density of both external and posterior limbs of the left internal capsules.
