- W2004391301 abstract "骨髄細胞には血管内皮細胞, 心筋細胞, 平滑筋細胞などの心血管系構成細胞の幹細胞が含まれる. さらに, 骨髄細胞自身が強力な血管新生因子であるVEGFやbFGFを分泌する. 骨髄細胞から幹細胞を含む単核球分画を単離し, 虚血組織 (虚血下肢・心筋) に移植すると毛細血管が増生し血行再建・機能改善が見られた. 新生血管には移植骨髄細胞が取り込まれ, 骨・軟骨への分化は観察されなかった. 動物実験での基礎データをもとに倫理委員会での承認を受け, ヒト虚血肢 (ASO・バージャー病) に対して自己骨髄単核球細胞を利用した血管新生療法 (Trial for Therapeutic Angiogenesis Using Cell Transplanatation) (TACT) を久留米大学・自治医科大学と共同で開始した. 2000年6月より, 2002年2月1日までに外科的・内科的治療によっても血行再建の認めない患者45人の虚血下肢 (Fontaine 3～4度) に対して自己骨髄細胞移植を randomised, double-blinded にて実施した. 対照細胞は末梢血単核球を用いた. 全身麻酔下で自家骨髄液約500CCを採取したのち骨髄単核球を分離し, 約10億個の細胞を虚血肢の筋肉内40箇所に分割注入した. 45人中31人で下肢の血圧が1月後には10mmHg以上上昇し, トレッドミル歩行距離は約2.9倍以上増加し, 下肢の疼痛は45人中39人で消失した. 下肢潰瘍は31人中27人で完全に治癒した. 腓腹筋注射部位の炎症・浮腫は認めず, 血中のVEGF・FGF濃度, 単核球細胞数は変化しなかった (Lancet 360: 427-435, 2002). 骨髄単核球細胞には骨髄間葉系・造血系幹細胞から分化する心筋幹細胞も含まれる. 家畜ブタを用いて慢性虚血心筋を作製し, NOGA3次元解析により冬眠虚血部位を同定しカテーテルを用いて細胞移植をおこなった. 全例で心機能改善・虚血域の縮小がみられた. 昨年12月19日に実施された難治性狭心症患者への開胸下移植では狭心痛の著明改善 (CCSクラスIVからI), 心筋虚血部の壁運動改善 (シンチ, エコー, NOGA解析), 心機能改善 (LVG: EF43%から52%) が見られた. 血管造影では側副血行の増生は認めなかった. 不整脈の発現は半年間見られていない." @default.
