- W2004904370 abstract "A Is-year-old schoolboy presented to Casualty with a painful left knee after a footballing injury. While attempting to kick a trapped ball, he heard his knee ‘crack’ and it became locked in a flexed position. Forcibly straightening it produced a second ‘crack’ after which weight bearing was impossible due to pain. Examination revealed a dislocated, unstable knee with a large haemarthrosis, but no signs of neurovascular impairment. The dislocation of the knee was reduced immediately with the aid of Entonox and a plaster backslab applied. Radiographs were then taken (Figure I) which showed an anterior Watson-Jones III fracture (Watson-Jones, 19551, and a posterior Salter-Harris IV fracture (Salter and Harris, 1963). At urgent operation, the fractures were reduced and fixed with two lagged 4.5mm screws and a single 2.7~mm screw, A postoperative radiograph is shown (Figwe~). The patient was discharged 10 days after operation. He was poorly compliant with physiotherapy; 4 months after operation he was still using crutches and had a range of knee flexion of 0” to 85”. Manipulation under anaesthesia and removal of screws was planned at 5 months after operation." @default.
