- W2004995897 abstract "Adjuvant radiotherapy combined with 5-Fluorouracil-based chemotherapy has become the new standard after curative resection of high-risk gastric cancer patients. Besides many complications due to surgery, the addition of chemoradiotherapy as adjuvant treatment may lead to acute and late toxicities. Among the well known ones are nausea, vomiting, diarrhea, malabsorption, renal, and metabolic complications. One of the less known effects is the decrease of the pancreatic reserve resulting in glucose intolerance with possible diabetogenic effect due to incidental and unavoidable inclusion of the pancreatic tissue within the adjuvant radiation treatment field. Fifty-three patients with gastric adenocarcinoma were evaluated for adjuvant treatment after curative surgical resection. While 37 out of 53 patients received adjuvant treatment due to either serosal or adjacent organ or lymph node involvement, 16 patients without these risk factors were followed-up regularly without any additional treatment. These 16 patients served as the control group. Fasting blood glucose, hemoglobin A1c (HBA1c), insulin, and C-peptide levels were measured in control group 4 to 6 weeks after the surgery and after 1 year. In the study group, the same measurements were done 4-6 weeks after the surgery and 1 year after chemoradiotherapy has been completed. At baseline there was no difference in age, body weight, fasting blood glucose, C-peptide, and insulin levels between the control and the study group. In regard of fasting blood glucose and HbA1c, there was no difference at the end of the study compared to initial levels. Insulin and C-peptide levels, however, were lower in the study group at the end of 1 year. Intergroup analysis revealed that there was no difference in insulin and C-peptide levels at baseline and at the end of the study in the control group, but there was a statistically significant difference in insulin and C-peptide levels in the same time interval; both measurements were lower after 1 year (7.5 ± 6.0 vs. 4.5 ± 4.4 IU/L, p: 0.002 and 2.3 ± 0.9 vs. 1.56 ± 0.9 ng/ml, p: 0.001).
