Accurate diagnosis is important to rule out secondary causes like fistula, hemorrhoids, tumors, allergens, infections, and dermatoses. Management begins with reassurance and patient education. Use of all over-the-counter preparations, cleaning pads, soaps, and solutions other than water must be stopped. Poor hygiene is only rarely a contributing factor. Overzealous cleaning and rubbing must be stopped to break the itch/scratch cycle and keep the skin dry and clean. Eliminating coffee, tea, colas, beer, tomatoes, chocolate, and dairy products may help in ameliorating symptoms. Soothing creams may be used. Short-term topical steroid cream may be prescribed. A biopsy should be performed if perianal lesions do not improve at a follow-up visit. More than 80% to 90% patients will have a favorable outcome with this approach, although recurrences are common, especially if the condition is idiopathic. For recalcitrant disease, various treatments have been used, including hypnosis and methylene blue injection in the perianal skin.
