- W2005248896 abstract "We aimed to study how preload (umbilical vein flow, UVF) affects DV-PI and MPI index. To study the correlation of UVF with DV PI and MPI index in fetuses with TTTS and compare it with controls. 216 TTTS were included. 46, 31, 135 and 4 were in Q. stages 1 to 4. 81 fetuses from normal pregnancies of 20–24 weeks were considered as controls. UVF was measured in the intraabdominal portion of the vein, with less than 45° and use of angle correction. MPI was calculated for the left ventricule. There was significant difference in UVF and MPI index between recipients and donors (Mann-Whitney paired test). UVF had a significant negative correlation with DV PI in recipients (Spearman r = − 0.36 P < 0.001) and donors (Pearson r = − 0.346 P < 0.001). This was not present in control fetuses (P = 0.14). MPI index had no correlation to these other two parameters in Recipients, though there was a significant correlation with UVF in Donors (0.234 P = 0.005). This correlation of UVF with DV-PI in the recipients remained significant when stages 1 & 2 were analyzed separated to stages 3 & 4. In presence of tricuspid regurgitation in the recipient, the correlation between UVF and DV-PI became non significant (P = 0.049). In recipients, as there is negative correlation between UVF and DV-PI, at greater UVF, lower DV-PI, contrary to what could have been hypothesized as a response to overload. MPI index was not related to these preload parameters in different Quintero stages. When tricuspid regurgitation was present in R, there was a loss of correlation between UVF and DV-PI. This response of the fetal heart represents the atrial adaptation in compliance and contractility that allows anterograd flow to the ventricles. When tricuspid regurgitation was present, this adaptation was lost." @default.
