- W2005659430 abstract "我が国の中途失明疾患には，緑内障，糖尿病網膜症，加齢黄斑変性症（age-related macular degeneration：AMD）および網膜色素変性症などがある．これらの失明性疾患の多くは網膜細胞の変性に起因することが知られている．しかしながら，それらの病態の機序については十分には解明されていない．一方，最近これらの眼疾患の一部とアルツハイマー病（Alzheimer disease：AD）との関連が指摘されている．すなわち，AD患者において網膜機能の低下，網膜神経節細胞の減少および視神経変性が高率に認められている．さらに，AD患者における緑内障の発症率が高いことが明らかにされた．ADは認知機能の低下，人格の変化を主な症状とする認知症の一種であり，その発症には脳内のアミロイドβペプチド（Aβ）の凝集・蓄積が引き金となり，神経細胞の変性・脱落が惹起されると考えられている（アミロイド仮説）．現在，本仮説に基づいてAD患者の脳内におけるAβの産生または蓄積を抑える様々な治療法が試みられている．最近，我々は緑内障および糖尿病網膜症患者の硝子体液中のAβ1-42の著明な低下およびタウタンパクの上昇を見出した．これらの変化はAD患者脳脊髄液中の変動と類似していた．また，AMDにおける唯一の前駆病変と考えられるドルーゼン中の構成成分としてAβが高頻度に存在することが報告された．これらの知見はこれらの網膜疾患とADの間に共通の病態発症機序が存在し，とくにAβが網膜疾患の病態に深く関わっている可能性を示唆している．本稿では網膜疾患のなかでADとの関連がとくに示唆されている緑内障および加齢黄斑変性症について，治療の現状とその病態の発症におけるAβの関与並びにアミロイド仮説に基づいた治療の可能性について概説する．" @default.
