O f these reported deaths, problems with intubation have been a constant feature, al though the report covering the years 1988-90 contained only one death associated with intubation problems. Does this mean that there is no longer a problem with intubation in obstetrics? The past decade has seen a dramatic change in obstetric anaesthetic practice that may account for some, if not all, of this improvement. The reduction in deaths from aspiration of stomach contents may be partly due to better maternal preparation, with the use of non-particulate antacids and H,-receptor blocking agents, and partly to the increased ability of modern intensive therapy to treat iatrogenic insults. However, this decreased mortality may have been at the expense of long-term significant morbidity of which little is known. The reports of 1985-87 and 1988-90’~~ both detail deaths that occurred some time after delivery following a prolonged period of intensive therapy; these deaths may represent the tip of a morbidity iceberg. The widespread availability and application of m inimum mon itoring3 have allowed early recognition and rapid correction of many problems which in the past may have threatened life. F inally, the change from general to regional anaesthesia has obviously led to a reduction in intubation problems. Despite the reducing number of deaths associated with difficult and failed intubation, there is no place for complacency particularly as the current trainee anaesthetist will have only lim ited experience in providing general anaesthesia to the pregnant woman.
