- W2005700810 abstract "Prevalence of CN is increasing owing to advances in diagnostic imaging. Differential Dx of CN is important because of the presence of potential malignant or frankly malignant lesions. 1st-tier differential Dx includes distinguishing pseudocyst (PC) from CN, while 2nd-tier differential Dx involves distinguishing serous (benign) vs mucinous (potentially malignant or frankly malignant) CN. Although many recent articles have outlined specific values of cyst fluid analysis in diagnosing specific CN, cyst size as a potential variable has, until now not been evaluated. Aim: Determine potential differences in cyst fluid analysis based on size of CN. Methods: Over 8-yrs, 156 pts were referred with CN of the pancreas for EUS evaluation & cyst fluid analysis. Of these, 85 lesions underwent surgical resection (54 M, 31 W). Sx's at presentation included, abd pain (n = 34), incidental finding on imaging (n = 18), acute panc (n = 27), wt loss (n = 9), abn amylase, lipase, &/or LFTs (n = 6). Cyst fluid analysis included viscosity (VIS), amylase, CEA, CA 19-9 levels, cytology. Exclusion criteria: 1) Neuroendocrine tumors, 2) IPMT, 3) Bloody fluid aspirate, 4) <1.5 cc fluid, 5) Lack of surgical pathology, 6) Non surgical candidate. Statistics: chi square, analysis of variance. Pt evaluation included resection of CN & resection or drainage of PC. Differential Dx was accomplished by surgical pathology & cyst fluid analysis. Results: For benign/inflammatory lesions (PC & serous CN) there was no statistical significant difference among cysts ≤2 cm & those >2 cm with respect to VIS, amylase, CEA, & CA 19-9 levels. In contrast, pre-malignant/malignant lesions (mucinous cystadenomas & mucinous cystadenocarcinomas) showed significant difference among cysts ≤2 cm & those >2 cm with respect to CEA levels (345 vs 1195 for MCYa & 1850 vs 45539 for MCYa-CA). Conclusion: Results of cyst fluid analysis demonstrates no difference in pts with PC & serous cyst lesions regardless of size. Cyst fluid CEA levels differ significantly in pts with small (≤2 cm) vs large (>2 cm) mucinous cystic lesions & should be taken into account when establishing a differential Dx." @default.
