La prevalencia varía en función de la población de referencia, en general aumenta con la edad y a menor antigüedad de la hipertensión arterial (HTA) y disminuye al aumentar la severidad de las cifras de PA y a mayor número de determinaciones de PA en consulta. Los ensayos clínicos están divididos en relación con la benignidad de la misma; en general, estos sujetos tienen una afectación orgánica menor que los hipertensos ambulatorios (HTAA) y superior a los normotensos. Con alteraciones bioquímicas y metabólicas más próximas a la HTAA. En los pocos estudios de seguimiento de HBB la morbimortalidad es inferior en éstos compararado con los HTAA. En cuanto a la evolución natural de la misma no se puede predecir en base a la PA en consulta ni ambulatoria, pero un porcentaje importante de los individuos con HBB evoluciona a HTAA, no pudiéndose descartar que sea un estado prehipertensivo. Deberá tratarse en función del riesgo cardiovascular del sujeto, debiéndose realizar el seguimiento. Es de esperar que en un futuro no lejano más estudios prospectivos determinen el verdadero riesgo cardiovascular de la HBB. White-coat (or isolated-office) hypertension (WCH) is defined as persistently high office-based BP and 24-hour ambulatory BP < 130/80 mmHg. Its prevalence varies according to reference popu ation, in general increasing with age and briefer period from onset of arterial hypertension,and decreasing with severely rising BP levels and more frequent office-based BP measurements.Cinical trials are divided in relation to the benignity of the hypertension. In general, WCH patients exhibit lesser organ damage than those undergoing ambulatory hypertension,but more than that of normotensive patients.They also exhibit biochemical and metabolic abnormalites more c osely resembling those found in patients with ambulatory hypertension. In the few WHC follow-up studies which exist, morbimortality is lower as compared with patients with ambulatory hypertension. It can also be stated that neither office-based nor ambulatory BP measurement can predict its natural evolution, although a significant percentage of individuals with WCH eventually become patients with ambulatory hypertension,leaving open the possibility that it is a pre-hypertensive state. It should be treated in function of the patient’s cardiovascular risk, and follow-up is essential. It is hoped that prospective studies in the near future will determine the true cardiovascular risk of WCH." @default.
