- W2006115128 abstract "The pharyngeal flap is the most common surgical treatment for velopharyngeal insufficiency (VPI) at Montefiore Medical Center, University Hospital for the Albert Einstein College of Medicine. It is strategically constructed between the posterior pharyngeal wall and velum corresponding to the site of VPI during speech. Variations in velopharyngeal closure patterns among patients with VPI necessitate varying sizes of flaps and their sites of attachment. An accurate assessment of lateral pharyngeal motion is the key to the success of the operation. Based on the concept of matching the breadth of the preoperative residual gap between lateral pharyngeal walls (LPWs) with the postoperative width of the pharyngeal flap, our experience with flap application in 226 patients resulted in eliminating hypernasality in 217 or 96.1%. Hyponasality occurred in 14 patients (6.2%) with subobstructing flaps and in 9 (4.2%) with wide flaps. This complication in children tends to resolve with general body growth by enlargement of the lateral ports, without loss of functional competency by the lateral pharyngeal walls. Residual hypernasality is corrected by a “patch flap”" @default.
