- W2006156251 abstract "P758 Aims: Basiliximab, a chimeric monoclonal antibody directed against the IL-2 receptor, is used for prophylaxis of acute rejection in organ allograft recipients. However, pharmacokinetics/pharmacodynamics in children and in adults are different, with children having a greater exposure to basiliximab at the recommended dose. This in vitro study compared the sensitivity of peripheral blood mononuclear cells (PBMC) from children, adolescents and adults to physiologically relevant concentrations of basiliximab. Methods: Blood was obtained from 29 volunteers: children, age 5-11 (n: 9); adolescents, age 15-17 (n: 10); and adults, age 26-42 (n: 10). PBMC were purified and CD25 and CD3 surface density measured. Proliferation in response to mitogen stimulation (PHA, 1 ug/ml) and to mixed allogenic lymphocytes (one-way MLR) was measured in absence or presence of different concentrations of basiliximab (0,2 ng/ml to 1 ug/ml). Additionally, the generation of cytokines IL-1, IL-2 and IL-8 were determined by ELISA in the supernatants of these PHA or MLR stimulated cultures in the absence or presence of basiliximab (1 ng/ml or 5 ng/ml). Results: The median relative fluorescence intensity, corresponding to CD25 cell surface density of resting PBL, was slightly increasing with age. In children was 10.4±6.3, in adolescents 15.7±12.0 and adults 19.0±11.4. The average relative fluorescence intensity, corresponding to CD3 cell surface density of resting PBL, was similar for all three groups. That of children was 70.2±12, adolescents 66.6±6.0 and adults 76.9±16.7. Basiliximab inhibited PHA or allogeneic driven PBMC proliferation similarly in children, adolescents and adults. IL-1 and IL-8 production after PHA or allogeneic stimulation were also similar in the three groups and were not inhibited by basiliximab. No IL-2 was detected at 24 hr post PHA or allogeneic stimulation either in uninhibited or basiliximab inhibited samples, in adults, adolescents or children, suggesting a possible consumption of IL-2 by the cells. Conclusions: PBMC from children, adolescents and adults are similarly modulated in vitro by basiliximab." @default.
