Given the tendency for postpartum weight retention and the implication of pregnancy in the development of obesity later in life, further study is required to determine appropriate gestational weight gain ranges for obese women. The purpose of this study was to examine the effects of reduced weight gain on pregnancy outcomes in obese and morbidly obese women. This population-based retrospective cohort study employed data from the Missouri Linked Birth-Death Certificate File. Pregnancy outcome data from 52,301 obese (body mass index, BMI = 29.0-32.9) and 56,793 morbidly obese (BMI ≥ 33.0) women with singleton births between 1990 and 2001 were analyzed using logistic regression. Compared with recommended adequate gestational weight gain (15-35 pounds), gaining under 15 pounds significantly reduced the risk of preeclampsia, c-section, and large-for-gestational-age infants in obese and morbidly obese women. The risk of small-for-gestational-age (SGA) infants was inversely related to weight gain in obese women [adjusted odds ratio (95% confidence interval) 1.6 (1.5-1.8) gain 10-14 pounds, 1.8 (1.6-2.0) gain 2-9 pounds, 2.4 (2.0-2.8) no weight change, 2.8 (2.1-3.7) loss 2-9 pounds, 2.9 (2.1-4.1) loss ≥10 pounds]. A similar trend was seen in morbidly obese women; however, the magnitude of the association was not as strong, indicating that the impact of gestational weight gain on infant birth weight is significantly different for this population. Risks for some adverse pregnancy outcomes can be decreased by reducing gestational weight gain in obese women; however, the negative effect of inadequate weight gain or gestational under-gain on SGA births is different for obese and morbidly obese women and cannot be ignored." @default.
