- W2006371586 abstract "Introduction: Practical depression tools are needed in clinical practice. The Patient Health Questionnaire (PHQ)-2 uses the first 2 items of PHQ-9. It is completed and scored quickly; but use in patients with HF is not well studied. Methods: Ambulatory (n=337) and hospitalized (n=294) patients with HF completed the PHQ-9 once. A subset of 120 ambulatory patients also completed the Beck Depression Inventory (BDI). Raw scores on BDI, PHQ-9 and PHQ-2, and depression classifications were compared using correlation statistics. Receiver operator characteristic curves were generated using PHQ-9 as a gold standard. Efficiencies were determined by area under the curve (AUC) analysis. Results: Correlation between PHQ-2 and PHQ-9 raw scores was 0.82; r2 0.67; P < 0.001 and between BDI and PHQ-2 raw scores was 0.76; r2 0.58; P<0.001. When PHQ-9 and PHQ-2 score AUC's were compared by depression level, only PHQ-9 and PHQ-2 scores for diagnosing mild depression indicated any difference between AUC (P = 0.004). When AUCs for PHQ-9 and PHQ-2 were compared by HF status- mild (ambulatory) vs. moderate-severe (hospital), there was greater consistency in identifying moderate-severe depression in ambulatory compared to hospitalized patients.Tabled 1Comparisons of PHQ-9 and PHQ-2 AUCs between Ambulatory and Hospitalized HF PatientsAmbulatory HFHospitalized HFDepression LevelAUCConfidence LimitsAUCConfidence LimitsP value for differenceMild0.8680.830.910.8750.830.920.8Moderate0.9160.880.950.8740.840.910.52Moderate-Severe0.9600.940.980.8720.930.92<0.001Severe0.9560.920.990.9410.910.980.57Uses PHQ-9 as a gold standard Open table in a new tab Uses PHQ-9 as a gold standard Conclusion: PHQ-9 and PHQ-2 have similar diagnostic efficiencies; thus PHQ-2 can be used clinically to quickly diagnose depression and severity in pts with HF. Since PHQ-9 is more efficient (higher sensitivity), a PHQ-2 diagnosis of mild dep. or a moderate-severe diagnosis of dep. in hospitalized pts should be confirmed by a PHQ-9 assessment." @default.
