- W2006757919 abstract "Background and Rationale: In 1998, over 5 million people in the developing world were newly infected with HIV. Resources for HIV prevention are limited, and policy-makers could profit from tools that can predict the impact of alternative control strategies. Objectives: To further develop and apply the STDSIM model as a tool for decision support for HIV/STD control in developing countries. Methods: The microsimulation program STDSIM was written in C, and runs on Windows 95/98/NT. Results: STDSIM simulates the transmission of HIV/STDs through a dynamic network of sexual contacts. The network contains hypothetical individuals with life histories resulting from stochastic events occurring over time, e.g., birth, migration, prostitution contact, STD transmission, start/end sexual relationship, death. Characteristics (e.g., STD status) of the simulated individuals are aggregated to generate population-based model outcomes (e.g., STD prevalence). Output includes HIV/STD incidences and prevalences, number of partners, and network parameters such as concurrency and component size. Simulated interventions include delay in sexual debut, condom use, partner reduction, STD mass treatment, and improved health care. STD-SIM allows the user to specify the level of detail of the input, thereby supporting models ranging from simple to complex and from generic to specific. STDSIM has been used for theoretical studies on sexual behavior interventions, is being applied to Nairobi and Nyanza (Kenya), and will be used to compare STD control strategies among Mwanza (Tanzania), Masaka (Uganda), and Rakai (Uganda). Conclusions: STDSIM is a flexible tool that can be used both for theoretical studies on HIV/STD epidemics and for estimating the impact of HIV/STD control strategies in specific settings. A new object-oriented version of STDSIM with an arbitrary number of STDs is under development. Future extensions include homosexual contacts and needle-sharing networks." @default.
