Describe the three levels of postresuscitation care and when they should be applied.The transition from intrauterine to extrauterine life is extremely hazardous, with probably more radical physiologic adjustments required during and immediately following the birth process than at any other point in a human lifetime. The remarkable aspect of birth is that more than 90% of babies make the transition smoothly, with little or no assistance. The remaining 5% to 10% require some degree of resuscitation. Although the percentage requiring assistance is small, the actual number of babies involved is substantial because of the large number of births. The World Health Organization has estimated that nearly 1 million lives are lost each year to neonatal asphyxia.1 Furthermore, morbidity is high among infants who survive birth asphyxia.Understanding the physiology of perinatal transition, gaining expertise in the most effective ways to resuscitate affected babies, and disseminating that knowledge and associated skills to perinatal clinicians is a most important challenge for the health care system. Dissemination of resuscitation knowledge and skills is particularly important, as evidenced by statistics showing that, in contrast to a relative dismal outlook for older children or adults who have required resuscitation,2 the skillful resuscitation of a newborn frequently is successful and usually is associated with relatively low morbidity.3### The Evidence Evaluation ProcessNational guidelines for resuscitation of adults initially were recommended by the National Academy of Sciences in 1966.
