- W2007151726 abstract "Biochemical failure after radical prostatectomy is a problem increasingly faced by oncologists and primary care physicians. Salvage radiotherapy (SRT) is often employed and remains the only potentially curative intervention in this patient population. There are several predictive factors validated to predict outcome to SRT, including the pre-SRT PSA value, which presumably is due to malignant PSA. We sought to explore the PSA changes measured half-way through SRT and to test their predictive value. 59 consecutive patients were referred for SRT from 2004-2008, and 38 were deemed eligible for analysis. Eligibility criteria included: no hormone use, no clinical evidence of metastatic disease, and a PSA measurement half-way through SRT (median days drawn 27.5 from SRT start). Median SRT dose delivered was 63 Gy (61.2-81 Gy). Biochemical failure post SRT was defined as a serum PSA value of 0.2ng/mL or more above the post radiotherapy nadir followed by subsequent rises or initiation of systemic therapy. Median follow-up time was 14 months (1-57.5). 71% of the patients experienced a drop in PSA half-way through SRT (group1), 13% experienced no change (group 2), and 16% experienced a rise (group 3). The mean drop in half-way PSA was 38% in group1, and mean rise was 54% in group 3. Crude failure rates were 22% in group 1, 40% in group 2, and 50% in group 3 (p = 0.3). 2 year actuarial freedom from biochemical failure rates were 79.7% in group 1, 66.7% in group 2, and 62.5% in group 3 (p = 0.2). Although the median follow-up is limited, these data are provocative in suggesting that PSA measurements half-way through salvage radiotherapy appear to change markedly from pre-radiotherapy values. This may serve as a predictive marker to long-term outcome and warrants continued future prospective investigations." @default.
