- W2007172807 abstract "To investigate pregnancy and labor complications and outcomes in grand and great grand multiparous women. A retrospective population-based study comparing singleton deliveries between the years 1988 and 2007 was conducted. Women who delivered more than once during the study period were included only once, in the group that matched their last delivery. Parturients were classified into three groups according to their birth order: multiparous (2-4 deliveries), grand multiparous (6-9 deliveries), and great grand multiparous (10+ deliveries). Stratified analysis included multiple logistic regression models in order to control for confounders. The study population consisted of 62, 074 women, 79% (n=49, 069) were multiparous women, 16.6% (n=9, 902) were grand multiparous women and 5.0% (n=3, 103) were great grand multiparous women. A significant linear association was found between parity (in the three groups) and adverse maternal and perinatal outcomes such as placenta previa, placental abruption, malpresentation, labor dystocia, shoulder dystocia, cesarean delivery (CD), meconium stained amniotic fluid, post partum hemorrhage (PPH), maternal anemia (HB< 10), congenital malformations, low Apgar score, and perinatal mortality. Using multivariable logistic regression models, controlling for possible confounders, grand multiparity was found an independent risk factor for labor dystocia, 1st stage (OR = 1.5, 95% CI 1.3- 1.7, P< 0.001), and perinatal mortality (OR = 2.0, 95% CI 1.7- 2. 5, P< 0.001). Great grand multiparity was found an independent risk factor for labor dystocia, 1st stage (OR = 2.6, 95% CI 2.2- 3.2, P< 0.001), labor dystocia, 2nd stage (OR = 2.1, 95% CI 1.6- 2.8, P< 0.001), and perinatal mortality (OR = 2.5, 95% CI 1.9- 3.3, P< 0.001). Women with high birth order are at increased risk for adverse obstetric outcomes. The risk is higher for great grand multiparous women compared to grand multiparous women.
