He raises a number of concerns in relation to the conduct of the case and I will attempt to address these in turn. Firstly, he is concerned by the use of multiple nerve blocks for routine knee arthroplasty, and the high doses of local anaesthetic agents used in these techniques. Such techniques have been used successfully for many years, with doses in excess of those used on this occasion, and have been shown to reduce the requirement for opioids in the postoperative period [1, 2]. Secondly, he thinks it unlikely that this was a case of inadvertent intravascular injection and was more likely to be one of local anaesthetic overdose. It is well recognised that inadvertent intravascular injection of local anaesthetic can occur despite all the precautions designed to prevent such an occurrence [3] and that toxicity can occur with a wide range of doses [4, 5]. I would suggest that it is difficult to be absolutely certain that inadvertent intravascular injection did not occur in this case. Furthermore, Professor Wildsmith suggests that ‘…whenever large doses of local anaesthetic are in use, all the drugs and equipment which might be needed for the management must be immediately at hand’ and that perhaps more midazolam should have been given. In the situation described, the second anaesthetist and I were dealing with the patient in an ABC approach and elected to give the Intralipid as a precaution to prevent progression to local anaesthetic induced cardiovascular toxicity, after the airway and breathing were dealt with. I agree with Professor Wildsmith that further midazolam may have controlled the seizures, but in this instance elected to give the Intralipid first. The fact that all this was performed within 2 min meant that it was indeed, immediately at hand. Finally, to satisfy curiosity, the patient had a spinal anaesthetic with intrathecal morphine a week later, with no ill effect." @default.
