- W2007275514 abstract "Objectives: Most ART units are currently using Day 3 FSH as a screening tool to determine couple’s suitability for ART. But the potential value of this test as a screening tool is not fully evaluated. This study examines the potential value of day 3 FSH level in young (<35 years) women undergoing ART in predicting (1) Response to ovarian stimulation and (2) treatment outcome. Design: Retrospective study. Materials and Methods: 953 IVF/ICSI treatment cycles undergone by 674 couples with female partner less than 35 years at the time of treatment, during the period from January 1997 to June 1999 were recruited into the study. The end points used for evaluation of response to ovarian stimulation were: (a) Treatment cycles stopped due to poor ovarian response, (b) total number of follicles aspirated, (c) total number of oocytes retrieved and (d) E2 on the day of hCG administration. The fertilization rate, cleavage rate and the number of embryos available for transfer were also analyzed. The value of day 3 FSH in predicting successful outcome was also evaluated. The data were analyzed using Data Desk and Stata release 6. Results: 57 treatment cycles were stopped due to poor ovarian response and the risk of this occurring was significantly associated with increasing day 3 FSH values. The predicted percentage of stopped treatment cycles increased from 5.1% at day 3 FSH value of 6 to 11.4% at day 3 FSH 10 and 15.6% at day 3 FSH 13. (OR = 1.19, 95% CI: 1.08 to 1.31). Each one unit increase in day 3 FSH increased the odds ratio of stopping treatment by 19% (p<0.001). The higher day 3 FSH values were associated with lower E2 on the day of hCG (p<0.0002), lower number of follicles aspirated (p<0.0001) and oocytes retrieved (p<0.0001) but was not associated with fertilization rate (p<0.87 NS) nor cleavage rate (p<0.84 NS). The number of embryos suitable for transfer were lower in patents with high day 3 FSH (p<0.01). 205 treatment cycles resulted in successful pregnancies giving a pregnancy rate of 21.5% per cycle started and 25% per embryo transfer. The probability of successful pregnancy fell from 21.9% at day 3 FSH of 6 to 18.4% at day 3 FSH 10 and 16.2% at day 3 FSH 13 but was not significant (OR = 0.94, 95% CI: 0.87 to 1.02). The female age was negatively correlated with treatment success (p<0.036). Conclusion: Our study suggests that day 3 FSH is an excellent predictor of response to ovarian stimulation in IVF/ICSI treatment. It is not a predictor of treatment success. Considering pregnancy as the major outcome of treatment, the role of day 3 FSH as a screening tool in ART needs further critical evaluation." @default.
