As a neonatal clinical pharmacologist, I am intrigued to see that we are still in the process of exploring the optimal use of antibacterial agents, such as aminoglycosides. Highlighting the use of the Monte Carlo method is important for the readership of The Journal, because many pediatricians and neonatologists consider pharmacokinetics and clinical pharmacology the territory of hospital pharmacies, not of the treating physicians. I hope that this article sparks increased dialogue and collaboration. What I miss in the article is a broader approach to the problem. Although the authors focus on gentamicin as the most frequently used aminoglycoside in the US nurseries, had they taken the time to search for studies investigating other aminoglycosides, they would have found several articles on extended-interval dosing of tobramycin in neonates.2De Hoog M.
