- W2007292043 abstract "Patent urachus cyst is a rare umbilical anomaly that has an incidence of 2 in 100,000 live births. Prenatal detection is rare and most cases are initially suspected to be omphalocele because of cystic structure in the umbilical cord or pseudo bladder exstrophy. Usually, failure of urachal obliteration occurs as an isolated fetal anomaly and has a good postnatal surgical repair prognosis. We report the case of pathological urinary sequence in a male fetus leading to a mega bladder, a urachus cyst and an umbilical cord pseudocyst, diagnosed prenatally using 2D and 3D ultrasound techniques. A 27-year-old woman was referred to our center at 18 weeks for diagnosis of mega bladder and 2 umbilical cord cysts in a male fetus. 2D ultrasound examination revealed a typical 20 mm umbilical cyst and an extra-abdominal 30 mm cyst communicating with the vertex of fetal bladder through 5 mm small channel. The bladder was enlarged and a dilatation of the rest of the urinary tract was present in relation to posterior urethra valves. 3D and 4D ultrasound imaging showed that the fetus voided urine both through the defect and the urethra. Fetal MRI performed at 28 weeks confirmed that the cyst communicating with bladder was fulfilled with urine. Termination of pregnancy occurs at 30 weeks because of associated cerebral septal agenesis. Autopsy confirmed the presence of urachal mucosa and the catheterization of the ruptured urachal cyst was communicating with bladder. Few cases of urachus cyst prenatally diagnosed are reported in literature but to our knowledge, none were associated with urinary tract disease and none used 3D, 4D and Fetal MRI. Our case illustrated that when 2D ultrasound can only show structures, 3D and 4D can improve accuracy of the prenatal diagnosis with fluid movement. This could help pediatrician surgeon in prenatal to explain the couple neonatal surgical repair and plastic reconstruction." @default.
