We report the case of a 79-year-old male who presented with a painful subtrochanteric metastatic lesion in the right proximal femur, secondary to prostatic carcinoma. He had initially been treated with percutaneous cementoplasty without prior orthopaedic consultation. His clinical condition worsened rapidly with the development of a displaced subtrochanteric fracture 10 days later, when the patient obviously required internal fixation. This procedure was considerably complicated by the presence of cement plugging the medullar cavity. The growing popularity of percutaneous cementoplasty among oncologists and radiologists should not fly in the face of biomechanical realities.
