The optimal dose is not known. To investigate treatment efficacy and morbidity using Polidocanol sclerotherapy 2 ml or 4 ml for hydrocele or spermatocele we performed a study. If the fluid contained sperms we defined it as a spermatocele/epididymal cyst. METHODS A double blind randomized clinical trial using either 2 ml or 4 ml Polidocanol for sclerotherapy of hydrocele or spermatocele was conducted in three university hospitals from 1993–2005 and recruiting 224 evaluable patients. The fluid was evacuated and 2 ml or 4 ml Polidocanol was injected by an assisting nurse with no possibility for anyone else to see the amount injected. Three months later at follow-up morbidity was studied by a questionnaire filled in by the patient. Recurrence of fluid was clinically determined and generally retreated. Complications were also detected through reading the medical records. RESULTS After the first treatment cure was observed in 59% in the 4 ml group and in 47% in the 2 ml group (p=0.04). In the 4 ml group more patients than in the 2 ml group had complications (31% versus 18%, p=0.04). Complications were mostly of low or moderate intensity with no or occasional need for medication. After one to four treatments 210 out of 224 patients (89%) were cured and another 10 patients (5%) were left with small amount of fluid with no difference between the groups.In hydroceles/spermatoceles larger than 175 ml cure after the first treatment was observed in 58% in the 4 ml group and in 34% in the 2 ml group (p=0.012) without differences in complications between the groups. CONCLUSIONS Sclerotherapy using Polidocanol was in the present study an efficient treatment for hydrocele or spermatocele with 94% satisfactory results after one to four treatments. A dose of 4 ml was superior to 2 ml, particularly in celes with larger amount of fluid.
