- W2007806912 abstract "To estimate the role of ultrasound assessment before fetoscopic laser photocoagulation (FLP) for twin–twin transfusion syndrome (TTTS) in predicting intrauterine fetal demise (IUFD) after surgery. Fifty-five patients with TTTS underwent FLP in Japan. Fetal biometry and Doppler studies of the umbilical artery (UA), umbilical vein (UV) and ductus venosus (DV) were performed in the donor and recipient fetuses just before FLP. The absence of fetal bladder and the presence of hydrops were also recorded. The association between each results and the occurrence of IUFD were analyzed statistically. The study was approved by the Institutional Review Board and all patients gave informed written consent. The incidences of IUFD were 25.5% (14/55) in the donor fetuses and 12.7% (7/55) in the recipient fetuses. The causes of IUFD in 4 recipient fetuses were unexplained. In 52 fetuses except 3 fetuses with obvious reason for IUFD, no significant association between IUFD and each ultrasound parameters including the presence of hydrops, end-diastolic absent or reverse velocity of UA (UA-AREDV), absent flow of DV (DV-AF), pulsatile flow of UV (UV-PF) and weight discordancy were observed. The causes of IUFD in 12 recipient fetuses were unexplained. In 53 fetuses except two with specific cause of IUFD, each parameters including the absence of bladder, DV-AF, UV-PF and weight discordancy were not associated with IUFD statistically. Only UA-AREDV was predictive for IUFD (Multiple logistic regression; p = 0.016, OR; 18.9, 95%CI; 1.75–204.9). Our data suggest that preoperative UA-AREDV may be useful to predict IUFD of donor fetus after FLP." @default.
