- W2008069666 abstract "Sir: We read with interest the article by Turan et al. entitled “Reconstruction of Lower Half Defects of the Nose with the Lateral Nasal Artery Pedicle Nasolabial Island Flap” (Plast. Reconstr. Surg. 119: 1767, 2007). We found the technique described to be very good and useful for reconstruction of the nose. We think the vascular supply is not well understood. The authors explain that the arterial input of the flap is based on the lateral nasal artery branch of the facial artery. At the time the flap is raised, however, this artery is divided proximally. The question, therefore, is how can the lateral nasal artery provide irrigation of the flap if it is divided at its origin in the facial artery? The facial artery usually terminates as an angular artery close to the inner canthus and anastomoses with one of the end branches of the ophthalmic artery: the nasal medial artery. In those cases in which the facial artery ends as a lateral nasal artery or some infrequent variation,1 the upper third of the face is irrigated by either the ophthalmic or the infraorbital arteries.2,3 In both cases, there are anatomoses among the facial, infraorbital, ophthalmic, and contralateral facial arteries.4 Taking these anastomoses into account, we can point out that after the flap is raised, a reversed arterial flow is necessary to nourish the flap. This reversed flow comes from the internal carotid artery through the ophthalmic artery and is anastomosed with the angular artery or through the infraorbital or contralateral facial arteries. Maybe it would be useful to continue with the Doppler probe toward the inner canthus to obtain the signal of the entire artery, since it will ultimately be the vascular source of supply of the flap.
