- W20083831 abstract "When coronary vessels are maximally dilated, flow through them is linearly related to coronary arterial perfusing pressure over the physiological range. The flows are also affected by the left ventricular systolic pressure. At a mean coronary perfusion pressure of 12 kPa (90 mm Hg), an increase in left ventricular systolic pressure fromm 14 to 19.1 kPa (105 to 143 mm Hg) decreases maximal subendocardial blood flow by 30% and at a mean coronary perfusing pressure of 8 kPa (60 mm Hg) similar increase in left ventricular systolic pressure decreases maximal subendocardial flow by 50%. Subepicardial flows were not affected by changes in left ventricular systolic pressure. These findings are important in practice, as illustrated by the effects of right ventricular systolic hypertension on right ventricular myocardial ischemia. Furthermore, because subendocardial blood flow is entirely diastolic, the mechanism by which a raised systolic wall force impairs diastolic flow must involve an interaction between systole and diastole and may be related to intramyocardial vascular resistances and compliances." @default.
